Indirect hyperbilirubinemia (total bilirubin of 6.0 mg/dL, indirect bilirubin of 5.2 mg/dL).
Elevated LDH (3828 unit/L).
Urinalysis exhibited 3+ blood and 4.0 mg/dL urobilinogen.
A direct antiglobulin test was performed, which was negative for IgG and positive for complement.
Diagnosis: Cold antibody autoimmune hemolytic anemia (AIHA).
Cold antibody AIHA is caused by agglutinating autoantibodies against red blood cell antigens, which prefer a temperature of 3-4°C, agglutinating and hemolyzing in cooler parts of the body. It is usually secondary to infection. Treatment involves treating the underlying disorder, keeping patient warm and warming blood products prior to administering to patient.
