HE stained BM biopsy sections show marked erythroblastopenia.
Some giant erythroblasts show big intranuclear viral inclusion.
CMV immunostain is negative.
A few erythroblasts show positive nuclear and cytoplasmic immunostaining for parvovirus (as shown in the left picture).
Diagnosis: Parvovirus B19 infection.
Parvovirus B19, a single-stranded DNA virus of the family Parvoviridae and genus Erythrovirus.
It can cause:
- Erythema infectiosum (fifth disease) in children.
- Aplastic crisis, esp., in sickle cell disease and hereditary spherocytosis.
- Hydrops fetalis.
- Chronic anemia in AIDS and arthritis in some patients.
